Echo Lake failed to comply with regulatory requirements related to their Risk Management Plan including Off-site Consequence Analysis, Process Safety Information, Process Hazard Analysis, Operating Procedures, Training, Mechanical Integrity, Incident Investigation, Emergency Response, and Risk Management Plan (RMP) submissions as required by the CAPP. We discovered these violations during an on-site inspection of the facility on January 17, 2024.
