Case summary

On June 11, 2017, EPA conducted a compliance inspection at the Facility and determined that Northstar failed to comply with certain conditions for a permit exception under Ill. Admin. Code tit. 35 � 722.134(a)-(c) [40 C.F.R. � 262.34(a)-(c)]. Specifically, EPA found that Northstar: (a) failed to mark the start date of accumulation on two hazardous waste containers and failed to make labels visible for inspection for several containers of hazardous waste.; (b) failed to maintain a copy of its Contingency Plan at the facility; (c) failed to successfully complete an employee training program; (d) failed to conduct weekly inspections of the hazardous waste storage area; and, (e) failed to maintain a copy of the land disposal restriction notification for hazardous waste. EPA sent a Notice of Violation (�NOV�) to Northstar on May 4, 2016, listing the alleged violations. Northstar subsequently submitted information to EPA to demonstrate compliance with the requirements at issue and agreed to pay a civil penalty of $5,000 to resolve the alleged violations.
