Case summary
On June 9, 2016, an authorized EPA representative conducted a compliance inspection of the City of Bloomington Water Treatment Plant to determine compliance with the RMP. EPA determined that Respondent violated the following regulations: 1. Respondent failed to maintain the supporting documents for the alternate release.; 2. Respondent failed to compile the design information of the ventilation system in the chlorine room; 3. Respondent failed to address the hazards of the process in the process hazard analysis; 4. Respondent failed to address the stationary source siting in the process hazard analysis; and 5. Respondent failed to prepare a training record for each employee that has received the refresher training on the operating procedures. The civil penalty was $2,400.
