Respondent failed: to assure that process hazard analysis recommendations are resolved in a timely manner and that resolution is documented, to perform inspections and tests on process equipment consistent with applicable manufacturers recommendations, to correct deficiencies in equipment that is outside acceptable limits before further use or in a safe and timely manner, and to promptly determine and document an appropriate response to each of the findings of the compliance audit and document that deficiencies have been corrected.
