This enforcement action was filed in response to violations of CAA 112(r)(7) which were discovered during a Risk Management Plan (RMP) compliance inspection conducted on 10/18/2012. Violations were related to Process Hazard Analysis, inspection and testing of process equipment, and documentation related to compliance audits and the correction of deficiencies that were found. The facility also failed to update emergency contact information within 30 days of each change. There were also failures in obtaining and evaluating the contract owner/operator's safety performance and program as well as a failure to monitor the work of the contract owner/operator in fulfilling its obligations.
