Case summary

Based on the Risk Management Plan (RMP) compliance inspection conducted on 6/2/2011 and a review of additional information received by the US EPA subsequent to this inspection, the following violations to CAA 112(r)(7) were found at Whiting Oil & Gas Corporation's Highland, MI Central Processing Facility: 1.) failure to maintain documentation and records; 2.) failure to comply with the hazard review requirements set forth in 40 CFR Part 68.50; 3.) failure to ensure that each employee operating a process has been trained and tested competent in the operating procedures pertinent to their duties; 4.) failure to implement procedures to maintain the on-going mechanical integrity of their process equipment.
