Case summary

Each of K & L Ready Mix's three facilities located in Ottawa, McComb and Kalida, OH were inspected on 8/31/2011. Each site had diesel, sand, limestone, cement, slag and other hazardous chemicals over the 10,000 lb. minimum threshold level. Each site failed to submit Tier II Forms for calendar years 2008, 2009 and 2010. The first Tier II Forms for each site were submitted on January 9, 2012 for calendar year 2011. All three sites are in close proximity to bodies of water and/or underground reservoirs.
