Case summary
This enforcement action was filed in response to violations discovered during a Risk Management Plan (RMP) compliance inspection conducted on 9/14/2011. Multiple violations of CAA 112(r)(7) were related to documentation of process parameters such as temperatures, pressures, flows and compositions. In addition to these, the Process Safety Information failed to contain information related to process system design of relief systems, ventilation systems, safety systems, design codes and standards and material and energy balances for the processes. Respondent also failed to certify annually that their operating procedures are current and accurate and that procedures have been reviewed as often as necessary.
