On 11/23/2009, Kenyon failed to immediately notify the NRC of a release of 7,084 lbs of ammonia (RQ = 100 lbs). The release occurred when an employee driving a tractor pulling a nurse tank swerved to avoid a collision with another vehicle. The tractor landed in a ditch and the nurse tank rolled over - causing the ammonia to be released.
