The facility failed to comply with CAA 112(r)(7) Program 3 requirements in the following areas: 1.) failure to revise emergency contact information in its Risk Managemet Plan within a month of any changes; 2.) failure to timely update and adequately revalidate the process hazard analysis for its refrigeration system; 3.) failure to include the operating limits within its operating procedures; 4.) failure to adequately implement the written procedures to maintain the on-going integrity of the process equipment; 5.) failure to promptly address all the findings of their compliance audit; 6.) failure to prepare an incident investigation report at the conclusion of its investigation.
