Case summary
On 2/17/2009 a fire in a bagging unit at Advanced Fiber Technology caused the release of approximately 77 barrels of paraffinic oil into the Sandusky River over a two-day period. While investigating this release it was discovered that AFT failed to submit MSDSs and Tier II forms for ammonium sulfate, monoammonium phosphate, 5 mol borax, boric acid, limestone, zinc sulfate and mineral oil to the SERC, LEPC and local fire department for calendar years 2005 thru 2009 by the March 1st reporting deadlines. This enforcement action addresses the above listed violations.
