Case summary
System has to sample for total coliform bacteria, nitrate and nitrite and provide public notice for past monitoring violations. Systme met the requirements of the AO by collecting a total coliform bacteria sample during the fourth calendar quarter of 2007, and the first and third quarters of 2008, but failed to sample during the second quarter of 2008, but subsequently sampled for total coliform bacteria during the fourth quarter of 2008, and second and third quarters of 2009. System collected two routine total coliform bacteria samples during the third quarter of 2009 in August and incurred a maximum contaminant level violation for this quarter because both samples were positive for total coliform. The Ohio Environmental Protection Agency (Ohio EPA) collected required repeat samples, and system collected five routine samples in September 2009 which are required to be taken the month following an MCL violation. All five of the routine total coliform bacteria samples your water system collected in September 2009 were non-detect. System sampled for total coliform during the fourth quarter of 2009 and this sample was also non-detect. OEPA advised that system replaced faucets and water lines in both bathrooms. System failed to collect a nitrate and nitrite sample by October 31, 2007 as required by the AO, but collected a nitrate sample in January 2008 and a nitrite sample in July 2008. System also collected a 2009 nitrate sample in August but it was after Ohio EPAs May 31, 2009 sampling deadline. System eventually provided public notice for the monitoring and reporting violations cited in the AO, and also provided public notice for the second calendar quarter 2008 and first quarter 2009 total coliform bacteria monitoring and reporting violations, and the failure to sample for nitrate during 2007 and by OEPAs May 31 sampling deadline during 2009. EPA issue system a letter terminating the AO 1/13/2009.
