Failed to submit a Form R for ammonia for calendar years 2002, 2003, and 2004. In response to this action they submitted reports. Failed to submit SERC, LEPC, and the City of Oregon Fire Department a completed Emergency and Hazardous Chemical Inventory Form (EHCIF) for urea for calendar year 2005. In response to this action they submitted EHCIF. In addition they failed to timely report a release of phenol to the National Response Center (NRC).
