Case summary
On September 13, 2004, the Dynaflex facility in Addison, Illinois, was inspected to determine whether the facility was complying with the Risk Management Program Regulations, 40 C.F.R. Part 68. The facility produces polyurethane seat cushions for the automotive and furniture industry. After the inspection, it was determined that Dynaflex had violated many areas of implementation of the RMP. U.S.EPA issues an Expedited Settlement Agreement on OCtober 28, 2004. The facility accepted the agreement, certified that they resolved all violations, and has paid a $1440.00 penalty to the U.S. Treasury.
