Case summary
The operator of the system has to do a Consumer Confidence Report and distribute it to their customers. It also must complete and submit the certification statement, explaining how the report was provided to the customers. System failed to prepare and distribute CCRs and submit certifications for 2001 and 2002. System will be permanently deactivate on October 15, 2003. FAO issued 9/23/03. I received notice on 10/2/03 via Fax from Randy Smith (OEPA) that Ladd Lands MHP will be permanently closed by October 15, 2003; system was deactivated on September 11, 2003 by OEPA. The owners will be expanding the campgrounds to include land where system currently exists. System has connected to Spring Valley Campground System. Withdrawal Letter issued on 1/16/04.
