Case summary
System has to sample its drinking water for the presence of total coliform bacteria for 2 consecutive quarters and provide public notice about past violations. It also must complete the PN certification form. System didn't take a total coliform bacteria sample for the 2nd quarter 1999 through the 2nd quarter of 2002. The system sampled during the 3rd quarter of 2002, and the 1st and 2nd quarters of 2003. The system also failed to notify the public of the above violations. System complied with all requirements of the AO, and R5 issued a letter terminating the AO to the system on 10/1/04.
