The system did not take a quarterly total coliform bacteria sample during the 4th quarter 2001, all for quarters of 2002, and the 2nd quarter of 2003, and did not notify the public of these violations. System complied with all requirements of the AO, and R5 issued a letter terminating the AO to the system on 10/6/04.
