Case summary
A 114 request to test was sent to Superior Aluminum on May 2, 2002. The request required Superior Aluminum to perform a capture efficiency and destruction efficiency test within 75 days from receiving the request. Superior Aluminum should have performed the request on July 15, 2002. Superior decided to build a permanent total enclosure to ensure 100% capture. Superior did not decide or have plans for the PTE until after the due date in the 114. Superior failed to comply with the Secondary Aluminum NESHAP. Superior failed to submit initial notifications and compliance status reports for all three furnaces. When Superior was required to perform the initial performance test, Furnace 3 was required to comply with the operating and monitoring requirements of the NESHAP. As observed by the Indiana Department of Environmental Management (IDEM) inspector, Superior had not installed a device to measure and record the weight of gaseous or liquid reactive flux injected into Furnace 3. Superior failed to record the inspections of the continuous lime injection system as required by the NESHAP. Superior also failed to maintain records required by the NESHAP. Furnace 3 performed the initial performance test in June 2001, which was 4 months late.
