Case summary

System failed to sample quarterly for coliform bacteria since early 1998, annualy for nitrate since 1997, and once for nitrite since 1993. AO requires system to notify the public by 5/31/02 of the failures to monitor, and to monitor for nitrate, nitrite, and coliform bacteria by 5/31/02, and coliform bacteria for the 7-9/02 quarter. System complied with the terms of the order except for not collecting repeat samles following the 8/22/02 positive total coliform bacteria sample result, and any of the 5 routine samples required the next month after a positive sample. The system notified it customers regarding these monitoring violations. The system collected 4 samples in October and 1 in December, and monitored during the 1st calendar quarter of 2003. On 5/6/03 R5 issued a letter to the system terminating the AO.
