System failed to prepare and deliver a year 200 Consumer Confidence Report (CCR) using drinking water data the system collected during 2000. The AO requires the system to prepare its year 2000 CCR , include a statement in the CCR that this report should have been provided by July 1, 2001, deliver the CCR to its customers by 4/15/02, and provide a copy of the CCR and a certification statement that includes the date and method it distributed the CCR to its customers to Region 5 and WDN by 4/30/2002. After the system eventually issued its 2001 CCR along with its 2002 CCR on 6/30/2002, delivered a notice to its customers that the 2001 CCR was late, and submitted the required certification statement, R5 issued a letter that terminated the AO on 5/5/2002
