System failed to collect quarterly coliform bacteria samples from late 1998 thru 2001, and annual nitrate samples for 1999 thru 2001, and provide public notice for these violations. The AO requires the system to provide public notice for past violations, and sample for coliform bacteria and nitrate by 4/15/02, and submit copy of the notice and sample results by 4/30; and sample for coliform bacteria during the 3rd and 4th calendar quarters of 2002 and 1st quarter of 2003, and submit sample results by the 10th day of the following quarter. The system met all the requirements of the AO, and on 3/13/03, R5 issued a letter terminating the AO.
