System failed to sample quarterly for coliform bacteria since early 1997, and annualy for nitrate since 1997. AO requires system to notify the public by 12/31/02 of the failures to monitor, and to monitor for nitrate and coliform bacteria by 1/31/02, and coliform bacteria for the 4-6/02 quarter. On 2/8/02 IDEM sent R5 a copy of IDEM's letter to to this church which inactivates it as a PWS because it connected to city water. On 12/19/02 R5 issued the system a letter withdrawing the AO.
