System failed to take any quarterly coliform bacteria samples during calendar years 1997-2000 and the first quarter 2001, annual nitrate samples for 1993-1995, and since 1996, and has never monitored for nitrite. The AO requires system to notify its customers regarding these failures, and to sample for total coliform bacteria during the 7-9/01, and 10-12/01 quarters, and nitrate and nitrite by 9/15/01. On 1/4/02 IDEM confirmed that it had inactivated this system from its PWS inventory because it did not serve enough people to be considered a PWS.
