Case summary

System failed to take any quarterly coliform bacteria samples during calendar years 1997-2000 and the first quarter 2001, annual nitrate samples for 1995-2000,and has never monitored for nitrite. The AO requires system to notify its customers regarding these failures, and to sample for total coliform bacteria during the 7-9/01, and 10-12/01 quarters, and nitrate and nitrite by 9/15/01. The system met the requirements of the AO except that it did not collect a total coliform sample during the 2nd calendar quarter of 2002, or any of the 5 routine samples required in 9/02 because the 8/5/02 sample was positive. The system did collect 4 repeat samples as required after the 8/5 positive sample, and notified the public regarding the 2002 violations. The system also collected its 4th quarter 2002 total coliform sample and collected a 2002 nitrate sample. On 3/10/03 R5 issued the system a letter terminating the AO.
