Case summary

System failed to monitor quarterly for coliform bacteria during all four quarters of 1997, the 1st, 2nd, and 3rd quarters of 1998, and all quarters after the 4th quarter 1998, and to monitor annually for nitrate during the years 1993 thru 1995, and 1997 thru 2000. The AO requires system to notify customers regarding these failures, and sample for total coliform bacteria during the 7-9/10, and 10-12/01 quarters, and for nitrate by 9/15/01. On 4/12/2002 IDEM notified R5 that this system had been deactivated from IDEM's PWS inventory on 12/19/01 because it did not serve enough people to be considered a PWS. On 4/26/2002 R5 issued a letter withdrawing the AO to the church.
