Case summary

SYSTEM FAILED TO MONITOR QUARTERLY FOR COLIFORM BACTERIA DURING ALL FOUR QUARTERS OF 1997, THE 1ST, AND 2ND QUARTERS OF 1998, ALL FOUR QUARTERS OF 1999, AND 2000, AND THE 1ST QUARTER OF 2001, AND ANNUALLY FOR NITRATE DURING 1993 AND THE YEARS 1996 THRU 2000. THE AO REQUIRES SYSTEM TO NOTIFY CUSTOMERS REGARDING THESE FAILURES, AND SAMPLE FOR TOTAL COLIFORM BACTERIA DURING THE 7-9/10, AND 10-12/01 QUARTERS,AND FOR NITRATE BY 9/15/01. The system met the requirements of the AO for monitoring total coliform bacteria and eventually met the requirements of the AO for monitoring for nitrate in 12/01, and public notice in 5/02. However, after the pastor left, the system failed to perform total coliform bacteria monitoring during 2 quarters in 2002, to collect any of five routine total coliform bacteria samples required in 7/03 because of a positive 6/25/03 routine sample, and to collect a sufficient number of repeat samples following the 6/25/03 positive sample. The system subsequently collected a 2003 nitrate sample, and total coliform bacteria samples during each quarter through the 2nd quarter of 2004, and provided public notice for the 6/03 and 7/03 violations. Although the system did not provide proof of publilc notice for the two quarterly violations in 2002, Region 5 sent the system a letter on 10/14/04 terminating the AO.
