Case summary

Water system failed to take annual nitrate samples and quarterly coliform bacteria samples, and notify the State and public of the violations. The AO requires the system to sample for nitrate once and for coliform bacteria for four consecutive quarters. System met the requirements of the AO for nitrate and total coliform bacteria (bacti) monitoring, and PN. However, after a change in pastors, the system failed to collect a 2002 nitrate sample, and 1st and 3rd quarter 2002 bacti sample, and to send EPA or IDEM documentation that it PN'd for these violations. The system subsequently collected a 2003 and 2004 nitrate sample, and bacti samples each quarter through the 2nd quarter of 2004. Although the system did not provide documentation that it PN'd for the 1st and 3rd quarter 2002 bacti monitoring violations, R5 issued the system a letter to the system terminating the AO on 10/13/2004.
