WATER SYSTEM FAILED TO TAKE ANNUAL NITRATE AND COLIFORM BACTERIA SAMPLES, AND TO REPORT THE VIOLATIONS TO THE STATE AND THE PUBLIC. THE AO REQUIRES THE SYSTEM TO NOTIFY THE PUBLIC OF PAST VIOLATIONS, AND SAMPLE FOR NITRATE ONCE, AND COLIFORM BACTERIA FOR FOUR CONSECUTIVE QUARTERS. The system sampled for nitrate and coliform bacteria as required by the AO except for the 2nd calendar quarter of 2001, and did send proof that it PN'd. The owner informed USEPA had sold the business at the beginning of 6/01, and MDEQ deactivated it as a public water system on 9/1/01.
