THE SUBJECT WELL IS NEITHER AUTHORIZED BY RULE NOR BY PERMIT. THEREFORE, RESPONDENT IS IN VIOLATION OF 40 CFR 144.11 FOR INJECTING INTO A WELL NOT AUTHORIZED BY RULE OR PERMIT.
