ACCORDING TO THE LETTER DETAILING THE VOLUNTARY DISCLOSURES, THE DISCOVERY OF THESE VIOLATIONS WAS MADE ON SEPTEMBER 25, 2000, AND THE REQUIRED INFO WAS SENT TO THE APPROPRIATE AGENCIES ON OCTOBER 11, 2000. FACILITY DEMONSTRATED THAT IT HAS MET ALL NIN CONDITIONS OF EPA'S AUDIT POLICY
