In #3A, #3B, #3C, #3D Boost Tank Room, and Substation 5, Anchor Glass had five PCB Transformers, TEW, T-42092, TEW, T-40638-2, TEW, T-40561-2, TEW, T-41727, and an Allis Chalmer, serial number 85451, in use, with combustible materials, stored within 5 meters of these PCB Transformers, violating 40 C.F.R. ��761.30(a)(1)(viii). Section 6(e) of TSCA, 15 U.S.C. ��2605, and the regulations found at 40 C.F.R. ��761.30(a)(1)(xii) provide that records of inspection and maintenance history shall be maintained at least 3 years after disposing of the transformer and shall be made available for inspection, upon request by EPA. Anchor Glass had two GE PCB Transformers, serial numbers G856237 and G856237, located in Substations 2 and 4. Anchor Glass failed to conduct visual inspections on these PCB Transformers at least once every three (3) months for leaks/spills as required in 40 C.F.R. ��761.30(a)(1)(ix) and failed to maintain inspection logs of the inspection findings for third quarter in calendar year 2002, violating 40 C.F.R. ��761.30(a)(1)(xii).
