'CAFO issued 9/30/2002, assessing a penalty of $15,679. Respondent shall make two payments of $7,839. The first payment is due by 10/30/02, last payment due by 11/30/02. Respondent did not submit, on or before October 17, 1987, or within three months after Respondent first became subject to OSHA''s MSDS requirements, an MSDS or a list including the required information about caustic soda, silver and sulfuric acid to the LEPC, the SERC and the fire department with jurisdiction over the facility. Respondent failed to submit a completed emergency and hazardous chemical inventory form for the listed chemicals to the SERC for calendar years 1997, 1998 and 1999, and to the LEPC and fire department with jurisdiction over the facility for calendar year 1997, by March 1 of of the year followinf the calendar year for which the report was required, and Respondent failed to include silver in the completed emergency and hazardous chemical inventory form submitted to the LEPC and the fire department with jurisdiction over the facility, for calendar year 1998. Respondent failed to submit Form Rs or Form As for silver to the Administrator of EPA or the official designated by the Governor of the Statre of South Carolina for calendar years 1996, 1997, 1998, by July 1, 1997, July 1, 1998, and July 1, 1999
