'CAFO issued 6/18/2002 assessing a penalty of $1,000. Respondent failed to submit a completed emergency and hazardous chemical inventory forms for diesel fuel and gasoline at its facility for calendar year 1998, 1999 and 2000 to the LEPC, the SERC and the appropriate fire department by March 1 of the year following the calendar year for which a report was required. Respondent did not submit, on or before October 17, 1987, or within three months after Respondent first became subject to OSHA''s MSDS requirements, an MSDS or a list including the required information about diesel fuel and gasoline to the LEPC, the SERC and the fire department with jurisdiction over the facility.'
