Case summary
On January 21, 2020 a Notice of Warning was issued to Custom Blended Solutions for the sale and distribution of pesticides with misbranding. Labels for the pesticides, Klear 1504 , Formula 1516 , and Klear 1597-B were found listing EPA establishment numbers that differed from the number assigned to Custom Blended Solution (#90628-WV-001). EPA establishment numbers are required to be on pesticide labels to identify the facility where the pesticide last underwent production. As a result, Custom Blended Solutions was cited for violation of FIFRA 12(a)(1)(E) - the sale and distribution of misbranded pesticides.
