Respondent failed to: (1) to provide the owners with the EPA-approved lead hazard information pamphlet and receive a written acknowledgement or certificate of mailing before each renovation; (2) retain all records documenting compliance with applicable work practice standards; (3) obtain firm certification prior to performing renovations; (4) remove all objects from the work area, including furniture, rugs, and window coverings, or cover them with plastic sheeting or other impermeable material with all seams and edges taped or otherwise sealed; (5) close windows and doors in the work area, or cover doors with plastic sheeting or other impermeable material, and/or cover doors used as an entrance to the work area with plastic sheeting or other impermeable material in a manner that allows workers to pass through while confining dust and debris to the work area; and (6) clean the work area until no dust, debris or residue remained at the completion of the renovation.
