HAZARDOUS WASTES ACCUMULATED AT THE FACILITY FOR GREATER THAN 90 DAYS AND THE RESPONDENT DID NOT HAVE A PERMIT TO STORE HAZARDOUS WASTES. IN ADDITION, EPA'S INSPECTION FOUND AN OPEN CONTAINER OF HAZARDOUS WASTE, NO CONTINGENCY PLAN, AND NO HAZARDOUS WASTE TRAINING PROGRAM IN PLACE.
