Respondent failed to submit nine (9) Periodic Reports on Continued Compliance as required by 40 C.F.R. 403.12(e), from June 2011 to June 2015. Respondent has not sampled and analyzed for TTO as required by 40 C.F.R 469.31 since at least June 2010. In addition, the Respondent exceeded the monthly average limit for lead for samples collected from August 2010 to June 2014, exceeded the maximum daily limit for flouride for a sample collected on March 28, 2012, the monthly average limit for flouride for samples collected in December 2013, and exceeded the monthly average limit for zinc for samples collected in December 2013, June 2014 and September/October 2014.
