Case summary

The Respondent sold a variety of dental cleaning products from its warehouse in Melville, New York. The four products sold included a Monarch Surface Disinfectant, Monarch Enzymatic Cleaner, Monarch Waterline Cleaner, and Monarch Surface Disinfectant/Dry Wipes Combo Starter or Refill Pack. Three types of violations were found during the inspection and via information request: sale of unregistered pesticides, including sale of a supplementally unregistered pesticide, sale of a registered pesticide for which claims made as part of the sale substantially differed from claims made in connection with the registration of the product, as reflected on EPA’s approved label, and importation of pesticides into the United States without filing the required Notice of Arrival form with EPA. The Respondent corrected the violations.
