Case summary

On February 3, 2011, the Respondent self-disclosed possible violations at its Creedmoor Psychiatric Center in Queens Village, NY under the Audit Policy for Clean Water Act (CWA), Safe Drinking Water Act (SDWA), and Resource Conservation & Recovery Act (RCRA). In general the violations included: Spill Prevention Control Countermeasure Plan (SPCC) under CWA; Underground Storage Tank (UST) inventory of stormwater injection wells under SDWA; large quantity generator issues under RCRA. Pursuant to the Audit Policy and based on information provided, EPA has determined that the facility meets the conditions of the Audit Policy for 100% elimination of the total gravity-based penalties. The Respondent corrected the violations that were disclosed on the February 3, 2011 Audit Policy request.
