On or about August 7, 2009, Respondent made an application of Vikane Gas Fumigant to a residential house in Mayaguez, PR. The Vikane label requires: a fact sheet must be provided to an adult occupant of the structure to be fumigated, food and medicines need to be removed, and signs must be posted in English and Spanish. The Respondent failed to follow the label. Penalty payment and incorporation of new signs.
