Case summary

The facility was inspected in July 2003 and the following violations were found: Failure to make a hazardous waste determination; failure to use a manifest to transport hazardous waste; failure to use a land ban certification; failure to minimize releases. Chemotherapy hazardous waste generated by Elmhurst contained 3 pollutants (see list above on form under environmental impact). Approximately 23 pounds of chemotherapy hazardous waste was generated on a annual basis for the 3 pollutants. Instead of shipping this waste for destruction at a medical waste incinerator, Elmhurst is now shipping all its chemotherapy waste (hazardous and non-hazardous) to be incinerated at a hazardous waste incinerator. Elmhurst is now accumulating its spent solvent and sending the spent solvent as hazardous waste to a treatment, storage and/disposal facility instead of dumping the solvent down the drain. The facillity is now handling its lead-acid batteries as universal waste instead of putting it in the regular garbage and is now handling its fluorescent light bulbs as universal waste instead of chrushing the bulbs.
