'This penalty order is a follow up to an administrative order which cited effluent violations, the respondent has failed to comply with effluent limitations, total dissolved solids, total suspended solids, temperature, zinc, and flow rate, contained in Table I.A. of its permit. The permittee failed to monitor in accordance with the Multi Sector General Stormwater Permit, failed to properly maintain laboratory records, failed to monitor and report flow, BOD and TDS in accordance with the permit and had unpermitted discharges.'
