UST VIOLATIONS RE: UPGRADE, RELEASE DETECTION AND FAILURE TO RESPOND TO A 9005. NOTE: name was Mr. T's Service Station but deleted and entered the proper respondent name with the submittal of the case conclusion data sheet. Small single facility that did not upgrade or have release detection. Mr Mikhailov was not served the 1999 complaint until March 2006. USTs were closed in June 2000. No contamination found. Penalty only settlement with no other injucntive relief required.
