Case summary

4/11/19 This is a combined complaint/CAFO with penalty to resolve Clean Air Act 112(r)(1) ( GDC ) violations found during an August 9, 2017 inspection at Respondent's facility located in West Haven, CT. Unsafe conditions found during the inspection included, among other things, wires on dead-man valves indicating an attempt to short circuit the protection these required valves provide, no step/bump protection on piping extending from the high pressure receiver, inadequate ventilation, inaccessible emergency shutdown valves, a valve on the roof leaking ammonia, and problems with piping supports, insulation, and corrosion. EPA alleged that City Line failed to (1) design and maintain a safe facility, taking such steps as are necessary to prevent such releases; and (2) minimize the consequences of accidental releases, should they occur. The CAFO resolving this matter requires City Line to pay a civil penalty of $40,600 and update its process hazard review for the refrigeration process.
