A. Overview of Information Collection
Title of Information Collection: Family Report, MTW Family Report,
MTW Expansion Family Report.
OMB Approval Number: 2577-0083.
Type of Request: Reinstatement with change.
Form Number: HUD-50058 Family Report, HUD-50058-MTW Family Report,
HUD-5008-MTW Expansion Family Report.
Description of the Need for the Information and Proposed Use: PIH
provides funding to public housing agencies (PHAs) to administer
assisted housing programs. The forms--HUD-50058 Family Report, HUD-
50058-MTW Family Report, and HUD-50058-MTW Expansion Family Reports--
solicit demographic, family profile, income, and housing information on
the entire, nationwide population of tenants residing in assisted
housing. The information collected through Form HUD-50058 is used to
monitor and evaluate the Public Housing, Section 8 Housing Choice
Voucher, Section 8 Project-Based Vouchers, and Section 8 Moderate
Rehabilitation programs. The information collected through Form HUD-
50058-MTW is used to monitor and evaluate PHAs participating in the MTW
Demonstration program which includes the Public Housing, Section 8
Housing Choice Voucher, and Section 8 Project-Based Vouchers programs.
The information collected through Form HUD-50058-MTW Expansion will be
used to monitor and evaluate PHAs designated as MTW pursuant to the
2016 Expansion Statute (Pub. L. 114-113), which includes the Public
Housing, Section 8 Housing Choice Voucher, Section 8 Project-Based
Vouchers, Local Non-Traditional Property-Based, and Local, Non-
Traditional Tenant-Based programs.
HUD collects tenant data to understand demographic, family profile,
income, and housing information for participants in the Public Housing,
Section 8 Housing Choice Voucher, Section 8 Project Based Certificate,
Section 8 Moderate Rehabilitation, and MTW Demonstration Programs. This
data also allows HUD to monitor the performance of programs and of the
PHAs that administer the programs.
With this reinstatement of an expired collection, HUD is updating
terminology for two line items on each form. Line 2w of Form HUD-50058
and line 2x of Form HUD-50058-MTW Expansion and HUD-50058-MTW are being
updated from ``Interim Reexamination reason'' to ``Interim
Reexamination/Mid-Cycle Changes'' to account for non-interim
reexamination types explained in attachment I.5 of Notice PIH 2023-27.
For each form, line 5j is being updated from ``Was the last passed
inspection an alternative inspection (Y or N)'' to ``Did PHA use
inspection flexibilities, e.g., alternative inspection or enter into a
HAP contract with non-life-threatening deficiencies? (Y or N).'' This
update provides clarity to PHAs complying with the use of alternative
inspections according to 24 CFR 982.406.
Respondents: PHAs.
