SUPPLEMENTARY INFORMATION:
Title; Associated Form; and OMB Number: Navy Insider Threat Report
Form; OPNAV 5510/423; OMB Control Number 0703-0092.
Needs and Uses: The Department of the Navy uses Office of the Chief
of Naval Operations (OPNAV) Form 5510/423, Navy Insider Threat Report,
to collect standardized information concerning potential insider
threats in support of the Navy Insider Threat Program. The collection
supports the Navy's implementation of Executive Order 13587, which
requires executive branch agencies to establish insider threat programs
to deter, detect, and mitigate insider threats and unauthorized
disclosures of classified national security information.
The form enables the Navy Analytic Hub to receive, assess,
integrate, and respond to reports of concerning behavior or other
information that may indicate a potential insider threat. Use of a
standardized form reduces the need for repeated follow-up
communications
[[Page 67828]]
and provides a consistent means to document relevant information,
including submitter information, available information about the
subject of the report, a narrative description of the concern, and any
known mitigation actions or previous reporting.
The form may be completed by individuals reporting observed
incidents or information within their knowledge, as well as by Navy
personnel gathering information in support of an insider threat
inquiry. For purposes of the Paperwork Reduction Act, the collection
accounts only for burden imposed on contractor respondents. Respondents
provide only the information available to them; they are not expected
to provide personal information about a report subject or information
concerning mitigation measures when they do not have access to that
information. Navy Analytic Hub personnel may supplement incomplete
reports using authorized Government sources and coordination with
appropriate command personnel.
Information collected through OPNAV Form 5510/423 is used to
support the Navy Insider Threat Program's mission to prevent, detect,
deter, and mitigate risks posed by malicious or unwitting insiders.
Affected Public: Individuals or households.
Annual Burden Hours: 25.
Number of Respondents: 100.
Responses per Respondent: 1.
Annual Responses: 100.
