Background

OPM is collecting information from tribal employers applying to
participate in the FEHB Program under 25 U.S.C. 1647b and subpart N of
5 CFR part 890. OPM defines ``tribal employer'' in 5 CFR 890.1402 to
include an Indian tribe or tribal organization carrying out at least
one program under the Indian Self-Determination and Education
Assistance Act or the Tribally Controlled Schools Act; or an urban
Indian organization carrying out at least one program under title V of
the Indian Health Care Improvement Act. A tribal employer must be
entitled to purchase coverage, rights, and benefits under the FEHB
Program for their tribal employees. Once a tribal employer elects to
participate in the FEHB Program in compliance with 5 CFR 890.1404, a
tribal employee who is considered a common law employee and meets FEHB
requirements for the type of employment (e.g., full-time, seasonal)
will become eligible to enroll in FEHB pursuant to 5 CFR 890.1405.
This collection consists of information identifying the tribal
employer and its representatives for FEHB purposes, key tribal
personnel, billing unit and bank account information, and a tribal
employer profile. Collection of this information is necessary to ensure
the validity of a program application and to facilitate enrollment in
the FEHB Program. The information is not published.
