ACTION: 30-Day information collection notice.

SUMMARY: The Department of Defense (referred to herein as ``the
Department'', ``Department of War'' or ``DoW'') has submitted to the
Office of Management and Budget (OMB) for clearance the following
proposal for collection of information under the provisions of the
Paperwork Reduction Act.
DATES: Consideration will be given to all comments received by October
28, 2026.

Type of Request: Extension.
Number of Respondents: 3,000.
Responses per Respondent: 1.
Annual Responses: 3,000.
Average Burden per Response: 15 minutes.
Annual Burden Hours: 750.
Needs and Uses: The Department of the Navy collects information
from individuals who request to embark on U.S. Navy surface ships or
submarines as part of the DV or FON programs. The primary purpose of
this information collection is to facilitate embarkation, determine
eligibility and qualification (including physical and medical
soundness), and ensure safety and security during the embarkation.
Affected Public: Individuals or households.
Frequency: On occasion.
Respondent's Obligation: Voluntary.
