The information collected on the DS-3053 is used to facilitate the
issuance of passports to U.S. citizens and nationals under age 18. The
primary purpose of soliciting the information is to ensure that parents
and/or guardians consent to the issuance of a passport to a child when
required by 22 CFR 51.28.

Methodology

The Department collects information from the parents or legal
guardians of U.S. national children when they complete and submit the
DS-3053. Passport applicants can obtain the form from an acceptance
facility/passport agency, manually sign it, and then have it notarized.
Alternatively, applicants can download the form on the Department's
website, fill it out electronically, and then print it for manual
signature and notarization. The form must be completed, manually
signed, notarized, and submitted along with the applicant's DS-11,
Application for a U.S. Passport.
